چکیده

We have read the article by Maxwell et al entitled “Benefits and Limitations of Macrotextured Breast Implants and Consensus Recommendations for Optimizing Their Effectiveness.” One statement in particular is strongly deserving of clarification. One of the paragraphs in the article begins with a discussion of the reported rates through six years of malposition (including rotation) for the Style 410 shaped cohesive silicone gel-filled implants. That discussion is followed by a statement that “In contrast, in a single-center study by Baeke of various Siltex microtextured implants, rotation rates were as high as 14%.” The authors did not disclose that the cited study by Baeke was not reporting rotation rates for the SiltexTM (imprintedtextured) surface among the available shaped (“anatomic”) cohesive silicone gel-filled implants. They were reporting results on anatomic saline-filled devices (specifically Style 2700 and 2900 implants) that were implanted between 1995 and 1999. The “various” implants used in the Baeke study were 317 anatomical saline-filled devices, including 118 Style 2700, 197 Style 2900, and 2 McGhan Style 163. Rotation rates of anatomic saline-filled devices do not contribute to a discussion of shaped (“anatomic”) silicone gelfilled breast implants. As noted by the authors, “in the 6-year follow-up of the core study of Siltex microtextured silicone gel devices, the rotation rate was 1.1% among patients who underwent primary breast augmentation.” Additional long-term follow-up results are now available from the same large, multicenter, prospective Core Study of Mentor’s (Santa Barbara, CA) MemoryShapeTM Breast Implants for primary augmentation patients, and they indicate a cumulative incidence of rotation by Kaplan-Meier analysis through 10 years of 1.5%.

A comparison of 500 prefilled textured saline breast implants versus 500 standard textured saline breast implants: is there a difference in deflation rates?

BACKGROUND This study provides the first large-volume (1000 implant) comparison of the deflation rates of Poly Implant Prosthesis prefilled textured saline breast implants versus a control group of Mentor Siltex textured saline implants. METHODS A consecutive series of 500 Poly Implant Prosthesis prefilled textured saline breast implants was compared with a consecutive series of 500 Mentor Si...

Textured surface breast implants in the prevention of capsular contracture among breast augmentation patients: a meta-analysis of randomized controlled trials.

BACKGROUND Capsular contracture is a common complication associated with the use of breast implants. Numerous randomized controlled trials addressing the efficacy of textured surface breast implants in reducing capsular contracture have yielded nonuniform results. This meta-analysis addresses the use of textured breast implants in the prevention of capsular contracture. METHODS MEDLINE, EMBAS...

Ten-year review of a prospective randomized controlled trial of textured versus smooth subglandular silicone gel breast implants.

Although textured silicone breast implants have been shown to reduce the incidence of capsular contracture, there is little evidence if this effect is maintained in the long term. It has been 10 years since the double-blind randomized trial in which 53 patients received either Mentor smooth (26) or textured silicone gel implants (27). Of the 14 patients who were not known to have developed a co...

Characterisation of breast implant surfaces and correlation with fibroblast adhesion.

INTRODUCTION Capsular contracture formation is a common complication following breast augmentation surgery. Breast implant shells have either a smooth or a textured surface. Smooth surfaces demonstrate a higher incidence of contracture formation. The 3-dimensional surface of textured implants is thought to disrupt contractile forces and reduce capsular contracture rates. AIM To investigate th...

Discussion: U.S. Epidemiology of Breast Implant-Associated Anaplastic Large-Cell Lymphoma.

BACKGROUND Breast implant-associated anaplastic large cell lymphoma (ALCL) is a distinctive type of T-cell lymphoma that arises around breast implants. Although rare, all cases with adequate history have involved a textured breast implant. The objective of this study was to determine the U.S. incidence and lifetime prevalence of breast implant-associated ALCL in women with textured breast impla...
